    That pendulum swings! Balanced is really a good word. We're moving toward a culture of expecting ease (laziness/early retirement is THE goal), but for the majority of human history, we've struggled for subsistence on some level. Do we know how to find balance as the population grows increasingly richer and we keep redefining needs vs wants? Consumer mentality plays into productivity a lot. How do we learn to curb our desires to realizing we have more than "enough". Our value is tied to more than our productivity; it's tied to our level of ease.

    I love the increased desire to NEVER be that therapist. Right? It's nice to learn that as a trainee and not carry these actions into the field. She told her husband? Wow. Part of me hopes she was just doing this for training purposes and it was a ruse.

    What I truly appreciate about this video is the distinction that you draw between getting stuff done and core values. You clarified this by pointing out that our intrinsic value as people is constant. I'm a relatively new therapist and I can't tell you how many times I've pointed this out to clients already. Yet I struggle to see it for myself. Right now, I'm putting the finishing touches on taking some leave for career burnout and this video is resonating with me big time. You rock. Seriously.

    Been gearing up to begin my masters in clinical counseling and, by far, the most daunting aspect of licensure for me appears to be the worry of how am I to work on these 3000 hours on the side. It doesn’t seem feasible to give up my full time job that keeps a roof over my family’s heads to get the hours done faster vs hoping to find the hours outside of my current job and pays anything at all for the time. I can’t imagine nurses going through a similar process, though the stakes for them tend to be higher in their profession.

    Hi Marie - Thank you for this video on a topic near and dear to my heart and I really appreciate your value-based perspective. I would like to add an additional perspective for folks to consider: I just passed my 1 year anniversary in private practice having worked for over 10 years in college counseling. As now a mid-career psychologist who has to manage chronic illnesses that affect energy levels and concentration, the idea of starting a private practice was frightening because of the messaging associated with the hustle culture. How would I ever live up to those standards? As I've learned more about the history of the term 'productivity', I found it interesting that our culture today has taken this concept that started as a descriptor of factories during the industrial revolution and now applies it to human beings. (I could go on a tangent here about being human beings and not human doings and the rise of people talking about burnout...). When I think about a term applied to machines being applied to humans, I get frustrated because as humans we are not machines. I've spoken with a lot of clients over the last several months who struggle with self-esteem because of not "being productive enough", and together we've developed an understanding of what it takes for factories and machines to actually function and be productive. It's called maintenance. If applied to human beings, I would call that self-care. To your point about re-defining productivity to fit with my own values, I focus on what I need to do to run a business - which is successful and allowed me to quit my job of 8 years last month! - while being compassionate and respectful of the needs I have as a human being so I can maintain my energy levels and enjoy life while making a living. And - a late gratitude to you - when I started thinking about private practice 4 years ago, I watched every video you had on how to do so and they were so very very helpful and I've recommended your channel to others you have wanted to figure out if private practice life is for them. -Hege.

    Appreciate that you spoke on this topic, this came at a very good time as a new therapist who, in my opinion, is tying up productivity and worth together. This has been my very first full-time job, and for quite literally 90% of my life at this stage, the foot has been squarely on the gas pedal; school, work, school, work, school, work. It was validating to hear that the introspection I've been doing regarding whether or not I should keep blazing towards my eventual end goal of private practice or being intentional about rejuvenation is from a good, mindful place. I would be lying if I said I knew the answer for myself at this point, but this gave me some much-appreciated clarity. Thanks for the great content!

    I felt a healthy balance starting my private practice and having flexible hours to prioritize family time. But alas my business is not sustainable so I'm looking at starting a 2nd job which doesn't align with my goals but I feel that I have no choice as I have kids to support.

    I think you hit the nail on the head when you said that people tend to tie up their worth with their productivity. It takes time and experience to build up a level of self confidence that does not require you to "prove" your worth through your actions. Just as it takes time to learn how to direct your energy toward the things you don't want to do but have to do, rather than avoid it. Like you, I'm privileged enough to work part time. The biggest difference from my life previous to this is that now I don't have to force myself to keep up with the work load each and every day. Yet when I do the work, I feel great because I actually have free time to enjoy. I've also noticed my goals getting smaller. Rather than devoting my time to something grandiose while feeling like I'm never going to reach it, it's enough to send those work emails and call it quits. The work gets done either way. I don't think my life has ever been better.

    An interesting topic, made me think. I've been a translator and done other jobs for 27 years, and I've been a psychologist for 9 years. For some reason it seems much harder to hustle as a psychologist - it seems somehow wrong and obtrusive - like people should just look for my help instead of me looking for clients. Like being a psychologist is morally incompatible with being a business owner. Of course I KNOW intelectually it's not true, but a part of me finds all the marketing activities very difficult and uncomfortable.

      I too have this fear of failure of getting clients. Now, I feel like whoever shows up is fine. And yes, I can use my time elsewhere too. 2nd income, go have a baby, etc.

    Bachelors Degree 4 years, Masters 2, plus having to wait an additional 2 years even if you get your hours in for licensure to make 30 bucks an hour after 8 years of education is absurd. Plus you only get paid for 45 minutes of the hour you see a client… hard pass

    I once had a therapist, during my intake session, point out all the ways (or well, some of the ways) in which my previous process with therapy had been an absolute mess. She had read my entire dossier thoroughly, remembered details about the therapy that I had in early childhood that I myself had completely forgotten, said that she hoped she could provide a more integrated plan than I had previously gotten (I had had bits and pieces from many different therapists and groups addressing single issues but never the whole picture), and when I was describing a frankly traumatic experience with a group therapy that made me want one-on-one therapy instead said that that therapy sounded like a bad idea and her approach was very different. I left that session elated because, for the first time, I felt understood, taken seriously and my concerns were heard by a therapist who went above and beyond with putting in the effort to get to know both me as a person and my medical history. So I think Dr Akopian saying her New York doctor was a quack could've worked, but not in this way haha. (And she is right: Rebecca's doctor WAS a quack and prescribed her way too much medication. Rebecca does need medication but not THAT). Only tangentially related to the video haha, I mostly wanted to rave about the amazing therapist who had read my dossier! The whole thing! That's, frankly, insane! Unfortunately I could only stay with her for a year because I moved, but in that year, she made a massive change in my life and I will always be grateful. I've had many good therapists but only one like her.

    Okay, some context for the song here. I really hope you read this (I know it's long, sorry!) because this show is very complex and watching songs out of context will always lead to important information being overlooked or misinterpreted. This song takes place shortly after Rebecca had a suicide attempt, and Rebecca is in a state close to mania (I don't know if it's actually a manic episode, since I don't know enough about that to judge) which is why she is putting all of this pressure on a diagnosis to Fix Everything Immediately. The doctor comments on this immediately when Rebecca steps into his office, pointing out that she is very elated and asking her to calm down before he gives the actual diagnosis. I think the hopeful nature of the song is very realistic to Rebecca's experience in that moment, but it is also setting up that she is not right about a lot of things. Life after having a diagnosis will still be hard and it won't make the symptoms go away. Like you said, there are signs within the song that Rebecca is overly optimistic. This is definitely intentional! After this song, everything crashes down when Rebecca hears the diagnosis is BPD and googles it, learning it is a "difficult disorder"/stigmatised disorder that many therapists refuse to treat. This sends Rebecca into a spiral for a bit and she even denies having the disorder (thinking it must be misdiagnosed again). It isn't until after a while that she goes back to her therapist and learns to have a more healthy relationship with her diagnosis. So this show shows many aspects of getting a diagnosis: the positive and hopeful, the scary and upsetting, and eventually the balance of learning to see it as a tool to help you understand yourself. Only watching this one song gives an unbalanced view, because it only views one of those many aspects addressed in the show. This is also why I don't mind the show making light of hearing voices, because it shows Rebecca's mental state in the moment. She is unable to see how scary it would be to hear voices because she is in this manic state. The show doesn't suggest that actually hearing voices would be easy or fun. EDIT: Yeah! Like you said in your editorial note, this show shows both sides of splitting in regard to Rebecca's reaction to her diagnosis! That's the word I was looking for!

    This is great. Another important point is to make sure you and your client are seated at the same level. A friend never returned to a therapist who sat higher up on a desk chair while she was lower down on a sofa; she felt it as a power imbalance.

    As a therapist in training, who also has autism, this really gave me some good ideas. I'm thinking about going for a person-centered approach, with dual seating arrangements (or perhaps three?) and gradations of lighting, color and texture, allowing the client the freedom to choose what section of the office works best for them. I wouldn't have considered this, without first seeing your video. Thanks!

    Thank you for this video! My biggest office design issue is related to sound-proofing. I'm on my 4th office now, and every one of them has had thinner than ideal walls. I feel like I've done a decent job of dampening sound from getting out of my office (with sound machines, some hidden sound panels, and sealing), but is there anything I can do to prevent sound from getting IN my office from neighbors I share a wall with, preferably in an aesthetic way that won't make my office look like a recording studio? On the topic of in-person vs telehealth, about 90-95% of my clients have preferred in-person. I've also found that I can turn over WAY more potentials into long-term clients if we meet in-person for the first meeting, so I prefer in-person too.

    I once asked a counsellor to tell me about her garden in an attempt to pull myself out of a trauma flashback. She panicked and left me in the midst of it, she didnt realise she could have been very generic talking about sun and grass, i just needed help to not slide in my mind. She made it worse and acted like i was violating significant boundaries and being inappropriate and weird. I just didn't want to get lost in flashbacks

    I'm a landlord, and I like to include the option to sublet in the leasing contract, as it helps the tenant cover their back end, say if they're away for extended periods, or if they want to end their lease early, thus allowing a sublease to become an assignment. The tenant setting up a co-working space, which is what she's talking about here, doesn't sit well with me though. My space is already rented out well bellow market value. If she wants to use it to supplement her income in this way, I feel like I should be getting a piece of THAT pie.

    lol! $50-100! Try $400+ ...it's so expensive, I'm doing it but I totally understand why many people don't. Hair and makeup is another $250. In Portland Oregon 2024.

      It's an older video ;) Prices have gone up for sure. Might I suggest you don't get your hair and makeup done, so you look as similar as you would in-office as possible. It offers authenticity and saves you a buck!

    Update: I just learned today that the current status of Google Workspace necessitates more than one individual to be part of the Google Business Workspace account to have HIPAA compliance through it for your Google Voice number.
